Vision:
To seed a value-based preventive health culture in the hearts and minds of people who live and work in underserved, disadvantaged and undervaled communities. To give birth to a culture that no longer accept low value and low quality care as an acceptable norm. To lead community health outcomes, in a bold systemic shift in outcome health culture, direction, learning, management, structure, and performance that will result in healthier choices, healthier pathways and a higher quality of life.
Strategic Purpose:
By 2025, achieve 80-90% engagement among community stakeholders, health professionals, health care organizations and educational institutions in a community-wide journey from volume-based to value-based care while promoting and advancing the evolution of health care transformation, from an inpatient, volume-based sick-care system, to an outpatient, value-based preventive health system.
How? By identifying and acquiring the T-BACK$™ – Thinking, Behavior, Attitude, Communication, Knowledge and $kills needed to make a sustainable coordinated shift to population health management practices. Our strategy intersects perfectly, with implementing preventive health measures that help people live longer lives.
Mission:
To promote and advance value-based health science, value-based care education, value-based care wellness training, and value-based care certification throughout underserved communities and beyond. To take more proactive steps in preparing communities and today’s health workers, for tomorrows demands, and prepare future health workers, primary care providers and private practices for the new challenges ahead, to ensure they not only survive, but thrive in a changing health care landscape.
Goal:
To sustain the cultural and professional capabilities of health professionals and institutions, who serve underserved communities, to better meet workflow integration demands, and fiscal challenges confronting health care organizations, while at the same time serving the changing health needs of the diverse communities they serve.
