Your investment in our Care Healthfully™ T-BACK$ Learning Clinics™ will fund our ability to upgrade the health training landscape in underserved communities through our learning health clinics.
Our clinics will cover a wide range of topics that promote, advance and optimize preventive health, value-based care experiences, outcomes and payment models.
What our learning clinic will do is develop value-based care credentialing standards where none have existed before. Our learning clinics will help communities, health professionals, health care organizations and educational institutions, schooled in the fee-for-service model of care, transition their T-BACK$™ – Thinking, Behavior, Attitude, Communication, Knowledge and $kills into the new roles they must play in a value-based care landscape .
For instance throughout underserved communities in thfe five boroughs of New York City, NO ONE has adequately and effectively prepared communities, health professionals, health care organizations and educational institutions for the journey from volume-based to value-based care.
This neglect, has negatively impacted the quality of community health outcomes, the revenue generating performance of health professionals, and the star rating quality of local hospitals.
What we mean here is:
NO ONE has developed a clearly defined definition of VALUE with community stakeholders that includes standardizing the mix of elements composing it (quality? service? cost? outcomes? access?). This is needed to serve as the foundation on which to build a value-based care cultural system throughout underserved communities.
NO ONE has standardized a training system that promote and advance value-based health science, value-based care education, value-based care training and certifications throughout underserved communities.
NO ONE has aligned such a training system to meet the performance expectations highlighted in the Affordable Care Act and all the other federal guidelines that regulates value-based care experiences, outcomes and payment models.
NO ENTITY has accepted sole responsibility of establishing, sustaining and navigating a borough-wide journey from volume-based to value-based care, that will facilitate the evolution of health care transformation, from an inpatient, volume-based sick-care system, to an outpatient, value-based preventive health system.
Despite the fact that the shift to value-based care has already kicked into high gear:
- 30% percent of payments were tied to alternative payment ACO or bundled payment arrangements by the end of 2016.
- 50% percent of payments will be tied to alternative payment ACO or bundled payment arrangements by the end of 2018.
- 85% of all traditional Medicare payments such as Hospital Value Based Purchasing and Hospital Readmissions Reduction were tied to quality or value by 2016.
- 90% of all traditional Medicare payments such as Hospital Value Based Purchasing and Hospital Readmissions Reduction will be tied to quality or value by 2018.
Your investment will subsidize the cost of this training or enable us to charge lower fees making it readily available to communities, students and health care professionals who would otherwise be unable to afford it.
Our online and printed educational tools exist only because caring individuals understand the importance of our work and care enough to give us their support.
So please allow us to thank you in advance for your support.
