HEALTHUP4U™ is a free online preventive health service that empower people to play a greater role in optimizing their own health outcomes.
HEALTHUP4U™ also partner with primary care providers, ambulatory care centers and health plans to support compliance targets regarding Preventive Health Visits, Vaccinations, Screening, & Treatment that impacts the health of their clients in the following areas; Well Child Visits, Flu, STD’s, HIV, Cancer, Diabetes, High Blood Pressure, Mental Health & Substance Abuse, etc.
Individuals can also conduct online health assessment by reviewing all four areas that make up the fourteen (14) categories of social determinants of health.
After an online health assessment score, community stakeholders review the assessment to identify areas of growth opportunities. Then, they select one or two of the most sustainable lifestyle choices from each category of social determinants of health that will improve the length and quality of their life.
In doing so, they enter into a health partnership with Get Healthier Care Together Inc., and commit themselves to achieving and sustaining a predetermined aim or goal, that includes lifestyle and behavior choices that healthify, build and advance a healthier way of life.
In exchange, Get Healthier Care Together Inc., support the commitment of individual stakeholders by offering direction and helping them develop a personal development health plan that will measure and track their progress and celebrate their success on-line.
Stakeholders also receive on-line coaching that connect them with best practices and cost saving measures. Annually, Get Healthier Care Together Inc. will recognize deserving participants in a televised public forum.
Individual stakeholders receiving SURGE OF HEALTH Awards will be in the spotlight for having become health champions who made the greatest SURGE in healthifying their life.
Project funding will help us develop HEALTHUP4U™ and expand this preventive health tool to communities, students and health care professionals.
