Our Care Healthfully Mobile Health Clinics™, will fill the vacuum between clinical settings and health care services delivered by hospitals and community health centers. Our Interventions will extend our capacity and capability beyond hospital walls to address both medical and social determinants of health needs.
Our Care Healthfully Mobile Health Clinics ™ will bridge fragmented care delivery, disjointed care coordination and the care disparity divide between community, clinical services and social determinants of health.
Our Care Healthfully Mobile Health Clinics™ will produce significant cost savings through preventative health screenings, initiating chronic disease management and offering urgent care and social determinants of health intervention on demand.

- Increasing the number of Quality-Adjusted Life Years and symptom-free days.
Our Health Watch Patrol Unit will patrol underserved communities with the highest health risk and the most expensive health disease.
Our Health Watch Patrol Unit will produce significant cost savings in improving health outcomes among vulnerable groups experiencing asthma, hypertension and obesity. These units will bring health solutions early and often on the street where they live. Instead of waiting of diseases reach a chronic state beyond the reach of prevention.
Our “MyHealth Ride” Transportation Services will reduce and eliminate barriers associated with making timely doctor appointment visits, medication pick-up and delivery from the lowest costing pharmacies and health exercise participation from the lowest costing fitness gyms.
Qualifying “MyHealth Ride” participants will be able to go online and schedule their pick-up and drop-off appointments with 24 hour notice.
